ADVANCED MONITORING

Build stability

before crisis.

Practical tools. Real accountability. Stronger community transitions.

Supporting safer, more stable transitions from custody to community.

ADULT OFFENDER GROUP

AOG

Structured accountability. One practical next step.

EARLY INSIGHT

Signal Tracker™

Weekly check-ins on housing, employment, stress, and support.

FUTURE DIRECTION

Housing

Developing pathways to stable housing.

SEE EARLIER. ACT EARLIER.

Signal Tracker™ provides a weekly self-reported view of housing, employment, stress, and support—helping partners notice changing needs and identify useful next steps.

Adult Offender Group (AOG) turns reflection into a documented Next Right Step: one action, one owner, one target date, and evidence of completion.

Together, these tools are designed to help partners notice emerging needs and support follow-through. They complement supervision and treatment.

Evidence & Evaluation

Pilot implementation and independent evaluation are being developed with correctional and community partners.

MISSION & VALUES

Building the infrastructure for stability before crisis.

Accountability

Clear commitments, ownership, and follow-through.

Practical Action

Focus on useful next steps and barriers that can be addressed.

How the model works

Notice changing needs

Brief weekly self-report.

Create accountability

Facilitated reflection and clear commitments.

Connect to action

One practical next step and appropriate support.

Document progress

Review follow-through and learn from implementation.
